The humphead wrasse is the largest living member of the wrasse family and one of the most commercially valuable reef fishes in the world, a combination that has placed it at the center of a conservation crisis across the Indo-Pacific. Slow-growing, long-lived, and naturally rare even in healthy reefs, the species changes sex during its life and aggregates in predictable spawning groups — life-history traits that make it acutely vulnerable to targeted fishing. As a roving predator of hard-shelled and toxic prey, it helps regulate populations of invertebrates and small fishes that few other reef predators will eat [IUCN 2021]. Demand from the live reef food fish trade, which prizes the fish for high-end restaurants in East and Southeast Asia, has driven steep declines across much of its range [Sadovy de Mitcheson et al. 2003]. Its recovery story is now closely tied to enforcement of international trade controls and the protection of the reefs it depends on.
Biology and Identification
The humphead wrasse is unmistakable among reef fishes. Adults can exceed 2 metres in length and weigh more than 190 kilograms, making it the largest extant wrasse [Sadovy et al. 2003]. Mature individuals develop a prominent bulbous hump on the forehead, thick rubbery lips, and a maze-like pattern of fine lines radiating from the eyes. Coloration shifts with age and sex, ranging from dull green or reddish-brown in juveniles to vivid blue-green in large terminal-phase males.
The species is exceptionally long-lived, with documented ages of at least 30 years, and it grows slowly relative to most reef fishes [Choat et al. 2006]. This combination of late maturity and slow growth means that populations recover only gradually once depleted. Females typically take five to seven years to mature, and the largest, most fecund individuals are precisely those targeted by the live fish trade.
Like many wrasses, Cheilinus undulatus is a protogynous hermaphrodite: individuals begin life as females and some later transform into males [Sadovy de Mitcheson et al. 2003]. Because males arise from the largest females, fishing pressure that removes big fish can skew sex ratios and depress reproductive output, compounding the effects of simple population loss.
Diet sets the humphead wrasse apart ecologically. Its powerful pharyngeal jaws allow it to crush hard-shelled prey such as molluscs, crustaceans, sea urchins, and even toxic animals like boxfishes and crown-of-thorns starfish that most predators avoid [IUCN 2021]. By preying on coral-eating invertebrates, the species is thought to contribute to the resilience of reef communities, although the strength of this role continues to be studied.
Habitat and Range
The humphead wrasse occurs across the tropical Indo-Pacific, from the Red Sea and the coast of East Africa eastward through Southeast Asia and the western Pacific to the islands of Micronesia and Polynesia [IUCN 2021]. It is closely associated with coral reefs, occupying steep outer-reef slopes, channels, and lagoons, with juveniles favouring branching corals and seagrass beds and adults ranging over deeper reef habitat.
Even in undisturbed reefs the species is naturally uncommon, typically occurring at densities of only a few adults per hectare [Sadovy et al. 2003]. This low natural abundance means that even modest fishing effort can rapidly reduce local populations, and the fish has been extirpated or severely depleted across large parts of Southeast Asia where exploitation has been heaviest.

Conservation Status
The humphead wrasse is listed as Endangered (EN) on the IUCN Red List, assessed most recently in 2024 [IUCN 2021]. The population trend is recorded as decreasing, driven primarily by overexploitation for the live reef food fish trade and the targeting of spawning aggregations and large breeding individuals.
The species is listed on Appendix II of CITES, which it joined in 2004 — the first commercially harvested reef fish to be added to the Convention [CITES 2004]. An Appendix II listing means international trade is permitted only with export permits certifying that the trade is legal and not detrimental to the survival of wild populations. The listing was a landmark recognition that an internationally traded reef fish required cross-border regulation, and it remains central to ongoing efforts to monitor and constrain the trade [Sadovy de Mitcheson et al. 2003].
Threats
Overexploitation for the live reef food fish trade. The dominant threat is targeted capture for sale, often live, to luxury seafood markets in East and Southeast Asia, where large individuals command very high prices [Sadovy de Mitcheson et al. 2003]. Because demand focuses on the biggest fish, fishing removes the most reproductively valuable members of the population and can collapse local stocks within a few years.
Destructive fishing methods. In several parts of its range the species has historically been captured using cyanide and other destructive techniques that stun fish for live capture [Sadovy et al. 2003]. These methods damage the surrounding coral reef habitat and kill non-target organisms, undermining the ecosystem on which the wrasse depends.
Spawning-aggregation fishing. Humphead wrasse gather in predictable spawning aggregations, and fishers who target these sites can remove a large share of a local breeding population in a single event [Sadovy et al. 2003]. The loss of aggregations is a recognised driver of decline for many reef fishes and is especially damaging for a slow-reproducing species.
Habitat degradation. Coral reefs across the Indo-Pacific are under pressure from warming-driven bleaching, coastal development, and pollution. Loss of healthy coral and seagrass habitat reduces both juvenile nursery areas and adult feeding grounds, adding a chronic stressor on top of direct fishing mortality [IUCN 2021].
What Is Being Done
The CITES Appendix II listing, in force since 2004, provides the primary international framework for regulating cross-border trade in the species. Parties are required to issue export permits backed by non-detriment findings, and the listing has prompted improved monitoring of trade routes and volumes [CITES 2004]. Enforcement remains uneven, but the listing gives range and importing states a legal basis to scrutinise shipments.
National measures complement the international controls. A number of range states, including Australia, have afforded the humphead wrasse legal protection or strict catch and export limits, and the species is protected within many marine protected areas across the Indo-Pacific [Sadovy et al. 2003]. Marine protected areas that effectively prohibit fishing can allow local populations to persist and rebuild over time.
The IUCN Species Survival Commission, through its Groupers and Wrasses Specialist Group, has played a central role in assessing the species, documenting the live reef food fish trade, and advising on management [IUCN 2021]. This work has informed the Red List assessments and supported the case for trade regulation.
Research and capacity-building efforts have also focused on understanding the species' biology, mapping spawning aggregations for protection, and developing aquaculture techniques to reduce pressure on wild stocks [Sadovy de Mitcheson et al. 2003]. Improved trade traceability and stricter non-detriment findings remain priorities for ensuring that any continued harvest is sustainable.

Support reef monitoring and citizen science. Divers and snorkellers can record sightings of humphead wrasse through reef-survey programmes such as those coordinated by Reef Check and other monitoring networks. These observations help scientists track where the species persists and where populations may be recovering.
Engage with trade and fisheries policy. Public support for robust implementation of CITES Appendix II controls, for protection of known spawning aggregation sites, and for well-managed marine protected areas strengthens the tools that govern the live reef food fish trade.
Make informed consumer choices. Avoiding consumption of humphead wrasse — and of live reef fish whose origins cannot be verified as legal and sustainable — reduces demand for the trade that drives the species' decline. Promote education and awareness. Sharing accurate information about the species' rarity, slow life history, and ecological role helps build the public understanding needed to sustain protective measures and to support the reef ecosystems on which the humphead wrasse depends.
